Child, Youth and Family Services

The Family Support team assists families caring for a child, youth or adult with a developmental disability who live at home.
Forms can be returned by fax, in person at the CLKD office or via email to Aimee Kim, Community Support Supervisor, akim@clkd.ca
We provide a wide range of person-centred, family directed support that is tailored to meet the needs and preferences of individuals and their families.

Some of the supports families can access include:

Referral and Application support - Assisting families to get connected to available community and regional supports and services

Service Navigation and Case Management - Acting as a central point of contact for families linked to multiple services or organizations

Accessing Funding and Tax Credits - Coaching families to complete financial and tax credit applications as well as providing advice and guidance on ways to utilize a variety of funding sources to access critical supports, respite and other services

Creating Community Connections - Engaging with individuals and families to identify social, recreational and volunteer activities and resources that may enrich their lives, build on their strengths and enhance their personal development

Educational Advocacy - Empowering families to effectively communicate with their school teams, participate in transition planning and understand educational support options and opportunities, including post-secondary pathways

Support and Peer Connections - Offering personalized outreach through home visiting and creating avenues to connect with peers in the community

Planning for the Future - Working with individuals and families to identify opportunities and develop long-range plans related to education, employment, housing, finances, decision-making and support services. Assist with preparing for and navigating the transition from Children’s Services to Adult Services

Specialized Programs and Services - Delivering a variety of courses and activities for individuals and families, including social awareness training (PEERS), Safety for Independent Living classes, All Kids Can Play workshops and the Youth Connections Summer Program
